The Ashland County-West Holmes Career Center uses an online forms and data management system.  The program called FinalForms, allows parents to complete required forms for your student.  The most exciting news is that FinalForms saves the data from year to year, meaning that you will never have to enter the information twice.  FinalForms also pre-populates information whenever possible for each of your students, saving you time.  

Returning students and parents/guardians from the 2023-24 school year, will just need to re-sign the different content areas.

All forms are required for parents/guardians and students to complete and sign electronically in order to be able to pick up the student schedule(s) and school issued computer device(s) for the 2024-25 school year.

Note: If your student is NEW to the district, please be sure to click the “New to District” checkbox when registering your student.
